2. Documents necessary in order to be considered for acceptance

b. Teacher recommendation forms in English (by math, science and language teacher)
c. SLEP or Jr. TOEFL score
d. Copy of passport
e. Immunization Report and Ear, Eye and Dental Report
f. All transcripts from current school.
i. Grades 9-12 must be sent by applicant to one of the following agencies for translation:

3. Upon acceptance
a. I-20 will be issued and given to host family or E&M to send to the student.
4. Registration will be completed by Michelle Gillis at E&M.
5. Tuition and book/activity fee
a. International families must wire funds to E&M for payment to the school.
b. This wired payment will include annual tuition and book/activity fee
6. A Georgia physician must transfer all International Immunization Records and Ear, Eye and Dental Reports to Georgia Forms BEFORE they can attend classes at Providence Christian Academy.
