Fun in the Afternoon
Statement of Purpose

The purpose of this program is to serve PCA families by providing quality educational, recreational, and social programs for students grades K-7 after the scheduled school day.

Program Goals
  • To provide homework/study time and assistance if necessary.
  • To provide a structured and safe environment for instruction in various sports designed to enrich and prepare students for formal sports programs.
  • To provide an opportunity for free play in age appropriate centers and activities.
  • To provide students an opportunity for supervised playground time in addition to the time allotted during the regular school day, thereby encouraging exercise and a healthy lifestyle.

Hours of Operation
FITA will begin at 2:45 p.m. (11:15 a.m. on early release days) and end at 6:00 pm. The program does not operate during weekends, or holidays such as Thanksgiving Break, Christmas Break, Spring Break, and Summer Break.

In the event of inclement weather or emergency closings, students must be picked up immediately. In these situations, refunds will not be given.

FITA ends promptly at 6:00 p.m. Late fees of $1.00 per minute will be assessed and are due by 6:00 p.m. the following day.

2011 – 2012 Enrollment
There will be Open Enrollment for all Providence Christian Academy students in grades K-7 throughout the summer and during the first two weeks of school. However, space is limited and students will be accepted on a first-come, first served basis.

Parents must complete and sign an enrollment form EVERY year. Parents are responsible for making sure that all contact, medical, and other pertinent information is updated. Parents must also sign a medical release permission form with a hold harmless clause. Providence Christian Academy Fun In the Afternoon Policies 2

***$25.00 Fee per student due at time of enrollment Program Fees
Program Fees may be paid on a weekly, monthly, or yearly basis. The following fee schedule will apply:

Weekly: $85.00 due on Monday (or first school day of the week) for the week in advance.

Monthly: $285.00 due on the last Monday of the month for the month in advance.

Annual: $2,600.00 due on August 1, 2011.

Occasional Participation Coupon
In order to provide for our families that may need the occasional assistance of FITA, we offer a coupon book good for five (5) visits for $125.00.

Guidelines for using this option are as follows

1. Students must have a completed enrollment form on file including the $25.00 enrollment fee.

2. Parents need to notify FITA that their child will be attending at least 1 day in advance.

** Note: Above rates include all Elementary/Middle School Early Release days. Regardless of payment schedule, there will no refund for any unused days.

Fee Acceptance
All payments must be given directly to Tom Duncan, Program Director. Do not send payments with your child. Receipts will only be given when cash payments are received. However, we are happy to provide you with tax receipts on an annual basis, and monthly statements for dependent care accounts.

Absences and Withdrawals
Our fees are based on a year-long commitment and are considered when planning for a superior program with qualified personnel. There will be no refunds if your child is absent.

If a parent withdraws a student from the program and has paid for the entire year, a prorated refund will be made as if the student was enrolled on a monthly basis.

Late Payments

Payments are due on the first day of the school week/month at the time of pick up or by 6:00 p.m. Failure to pay by this time will result in a $5.00 late fee, due on the next school day. If a child is absent from school, payment is due on the first day that the child returns. Providence Christian Academy Fun In the Afternoon Policies

Returned Check Fee
Returned checks will result in a $25.00 bank fee in addition to the $5.00 late fee. More than 2 returned checks will result in payments being required by cash, cashier’s check, or money order.

Student Pick-up Procedures
Students will be released ONLY to persons who are authorized as indicated on the enrollment form. Students must be picked up from the FITA classroom, and will not be sent to the front office/lobby area. Parents may update the pick-up authorization by adding/deleting names and contact information in writing. All persons authorized to pick up students will be required to present photo identification and sign out the student.

Student Illness, Accidents, or Injuries
If a student has a temperature of 100 degrees or higher, or presenting symptoms such as a rash, vomiting, diarrhea, or persistent sore throat, parents will be notified to pick up their child. The student will be separated from the other children to avoid contact and possible spread of illness.

Students who are absent from school cannot attend FITA that day.

If it is confirmed by a physician that students are exposed to communicable diseases, parents will be notified in writing as soon as we are notified.

Parents are encouraged to make arrangements for prescription medication to be administered by the school nurse whenever possible. However, if a child has an adverse reaction to prescribed medication, parents will be notified immediately. In addition, the child’s pediatrician or 911 may be notified by the Program Director.

Minor injuries will be addressed by the Program Director. If the Program Director deems necessary, the parent, the child’s physician, and/or 911 may be contacted. If necessary, Eastside Medical Center will provide emergency service to the student.

Parents will be informed of all injuries via an injury report form.

Personal Items
Children are discouraged from bringing any items to FITA that are valuable, sentimental, or fragile. Electronic devices such as cell phones, mp3 players, and digital/video games may be brought with the understanding that these items are the responsibility of the student. Damaged or lost items are not the responsibility of the FITA staff.
